Eatwell Farm Box Application Form

UPDATE: THE CSA IS CURRENTLY FULL. We are still accepting applications--filling out and submitting the below form will ensure you a spot on our waiting list. WE WILL CALL YOU WHEN A SPACE IS AVAILABLE, and you can give us your payment information at that time.

This form is intended for new members only. If you are renewing or re-starting a subscription (even after a long absence) please call us at 866-627-2465!

We have spaces available at many of our pick-up locations. Please check below to see if the location you desire is open. Note that we will provide the address of your site once you join the CSA!

Submitting this form does NOT automatically begin your subscription. We will process your application once we've received payment (you must contact us with payment information). Then we will send you a welcome email including confirmation of your pickup schedule and pertinent site information. Please call with any questions.

If you would like to receive a weekly email with what will be in the box the following week, email majordomo@list.dcn.davis.ca.us. Include only the words subscribe eatwell in the body of the message. Use plain text, and be sure to keep the subject line blank.

You can cancel your box or change your pick-up location by just calling 866-627-2465 by 10 a.m. the Friday prior to your next delivery.

When would you like to start? We need to receive your payment by Friday at 10 a.m. to start the following week.

Payment . . . We will contact you when a space becomes available for you. We're not sure how long this will take, since we have never had a waiting list before! We're projecting it may take around a month (it may be shorter than that, and it may be longer). When we call you, we will need payment before you can start. Paying by a VISA or MasterCard will ensure the fastest start. If you would like to send a check, write it to Eatwell Farm and note that it may take well over a week for mail to arrive here from the Bay Area. We will wait until we receive payment before beginning your deliveries!
